Job Summary:

R3 Health is a dynamic and fast paced healthcare environment. We are seeking a dedicated and holistic-minded Patient Care Coordinator to join our functional medicine practice. The ideal candidate will be passionate about integrative health, supporting our healthcare team in providing personalized care that addresses the root causes of illness. This role involves administrative duties aimed at enhancing patient wellness through a comprehensive approach to healthcare.

Job Requirements:

• Bachelor’s degree in a healthcare-related discipline
• Excellent interpersonal and communication skills, with a compassionate and patient-centered approach.
• Ability to multitask and thrive in a collaborative, patient-focused environment.
• Proficiency in computer programs, included but not limited to: Microsoft Office programs, adobe, google suite, and electronic health records.
• Previous experience in healthcare is preferred, though not required.

Patient Care Coordinator Job Responsibilities

• Follows all HIPAA policies and procedures. 
• Maintains highest levels of security and confidentiality for sensitive and/or personally identifying information (PII).
• Collaborates effectively with other administrative team members to establish a productive flow of communication and completion of tasks.
• Collaborates effectively with clinical staff to establish a productive flow of communication and completion of tasks.
• Coordinates any preparations required for assigned patients upcoming appointments, in-office or virtually.
• Assists in patient data collection, ensuring all necessary medical and demographic information is filed prior to initial onboarding appointment with medical provider.
• Transcribes medical provider communication with patients into medical notes on an as needed basis.
• Executes in-depth treatment explanation based on clinical recommendations from medical provider.
• Professionally addresses any administrative questions executive clients have in reference to treatment explanation.
• Communicates medical questions patients may have in reference to treatment explanation with medical provider and respond in a timely manner.
• Notate any written and verbal communication as it pertains to medical questions in the client’s EHR chart.
• Executes order fulfillment based on clinical recommendations from medical provider and treatment explanations
• Assists patients in organizing treatment protocol to create a seamless adoption practice. This can include organizing supplements for daily consumption, reconstituting medications, or pre-filling injectable syringes based on executive team provider’s medical recommendations notated in chart.
• Assists patients in scheduling follow-up appointments, lab draws, lab review appointments based on repeat frequency determined by medical provider.
• Assists patients in scheduling recommended in-office procedures, including monthly IV schedules.
• Establishes a role as on-going point of contact for assigned patients via phone call, text, email, zoom, google meet, etc.
• Acts as intermediary contact between established patient and refill department when a refill is requested.
• Monitors and performs current office protocols and clearly communicates with management if practices are outdated or inefficient.
• Stays current on topics in the functional medicine, biohacking, regenerative medicine space. This includes listening to podcasts and writing blogs on new trends on an as needed basis.
• Demonstrates a willingness to perform any task for the betterment of the practice.
